Communication Consent & Electronic Communications Agreement

By signing this Agreement, I authorize D&T Murphy and Associates, Inc., including its affiliated businesses operating at the Crystal Run Galleria complex in Middletown, New York, including Gold's Gym, SWING, and Parisi Speed School (collectively referred to as the “Company”), to communicate with me using the contact information that I provide. I understand and agree that the Company may contact me by telephone, email, text message (SMS/MMS), mobile application notifications, or other electronic means regarding my membership, participation, reservations, purchases, events, and other services offered by the Company.
Types of Communications
- Membership and account information
- Billing notices, receipts, and payment reminders
- Class schedules and appointment confirmations
- Reservation confirmations and updates
- Program registrations and reminders
- Facility hours, closures, maintenance notices, and operational updates
- Special events, leagues, tournaments, camps, clinics, and competitions
- New programs and services offered by Gold's Gym, SWING, or Parisi Speed School
- Promotional offers, discounts, member-exclusive specials, and marketing communications
- Health, fitness, performance, and educational content
- Customer service follow-up and satisfaction surveys
- Safety, security, or emergency notifications affecting members or participants
Text Messaging (SMS) Consent
By providing my mobile telephone number and signing this Agreement, I expressly consent to receive recurring text messages from the Company at the number provided.
- Message and data rates may apply.
- Message frequency may vary depending on my membership, participation, reservations, and interactions with the Company.
- My consent is not a condition of purchasing any membership, product, service, or program.
- I may opt out of marketing text messages at any time by replying STOP to any text message.
- I may reply HELP for assistance or contact the Company directly for support.
- Certain transactional or account-related messages may still be sent as permitted by applicable law.
Email Communications
I consent to receive emails regarding my membership, programs, reservations, account activity, educational information, newsletters, promotions, and special offers from Gold's Gym, SWING, Parisi Speed School, and D&T Murphy and Associates, Inc.
I understand that I may unsubscribe from promotional email communications at any time by using the unsubscribe link included in the email or by contacting the Company directly. Transactional and account-related emails may continue to be sent as necessary.
Updating Contact Information
I agree to promptly notify the Company if my telephone number, email address, mailing address, or other contact information changes so that important communications may continue to be delivered.
Privacy
The Company respects the privacy of its members and participants. Personal information will be collected, maintained, and used in accordance with the Company's Privacy Policy and applicable federal and New York laws. The Company does not sell personal information to third parties for independent marketing purposes.
Electronic Consent
I acknowledge that my electronic or handwritten signature below constitutes my affirmative consent to receive the communications described in this Agreement. I understand that I may withdraw my consent for marketing communications at any time; however, doing so will not affect the Company's ability to send communications that are necessary to administer my membership, participation, reservations, purchases, or account.
By signing below, I acknowledge that I have read, understand, and voluntarily agree to this Communication Consent & Electronic Communications Agreement.
